New Drug Class for Diabetes and Obesity Reaches Indonesia as Data Refine Diet Paradigm

Tirzepatide, a dual-hormone therapy, has been approved in Jakarta, while a mouse study and human survey suggest that cutting a single amino acid may mimic some metabolic effects.

Indonesian regulators have approved tirzepatide, a dual GIP and GLP-1 receptor agonist, for type 2 diabetes and weight management, adding to a class that already includes semaglutide and liraglutide in the country. The move comes as the International Diabetes Federation projects that 20.4 million adults in Indonesia have diabetes, a figure set to reach 28.6 million by 2050, and national health surveys show obesity prevalence climbing to 23.4 per cent in 2023. The fast-track authorisation by the national food and drug agency underscores the pressure to expand therapeutic options for metabolic disease.

Tirzepatide mimics hormones released after eating, boosting insulin secretion, suppressing glucagon and slowing gastric emptying. A separate line of inquiry, published in Cell Metabolism by researchers led by Valter Longo of the University of Southern California, tested a modified longevity diet in aged mice: vegan with fish two to three times a week, and restricted in the amino acid methionine, which is abundant in red meat. The mice consumed more calories yet shed fat mass while retaining muscle, and displayed lower IGF-1 and higher FGF-21 and GLP-1 levels — hormonal shifts associated with metabolic health. When the team compared these findings with dietary and health data from over 200,000 people in cohorts run by Harvard and the University of Toronto, higher animal protein intake was associated with roughly double the risk of type 2 diabetes.

In Jakarta, the licence holder, PT Anugerah Pharmindo Lestari, a unit of Zuellig Pharma, has said it will partner with medical organisations to manage education and distribution. Prof. Dante Saksono Harbuwono, head of the Indonesian Obesity Study Society, has stressed that obesity must be treated as a chronic disease, noting that certain drugs can induce epigenetic changes that help the body relearn metabolic patterns, potentially delivering up to 20 per cent weight loss. Yet the success of any therapy also depends on care delivery: a survey by the Swedish Diabetes Foundation found that in Kalmar County alone, 19 per cent of diabetes patients had not seen a diabetes physician in the past year, raising questions about continuity.

The immediate measure for tirzepatide will be post-market pharmacovigilance conducted by Indonesia's food and drug agency. Longo’s group cautions that while the observational human data are consistent with the mouse findings, the dietary intervention itself remains at the animal model stage; controlled human trials on methionine-restricted diets are still needed. Indonesian health authorities are meanwhile promoting a holistic approach that combines prevention, early detection and lifestyle change alongside novel pharmacotherapy.
